This page defines how AI incidents and unresolved failures should be escalated, and how governance boards enforce compliance, accountability, and structural fixes.
It ensures that no bug or bias can stay hidden, and that every systemic failure is linked back to a permanent WFGY solution.
When to use this page
- Incident or bug repeats despite local fixes.
- Users escalate “model is unsafe / biased / opaque” issues.
- Regulatory or audit deadlines require traceability.
- Stakeholders demand oversight board review.
Acceptance targets
- All escalations include ΔS, λ, and traceability logs.
- Time to escalation ≤ 48h from repeated failure.
- Governance board review within 7 days.
- Root cause mapped to permanent fix in Problem Map.
- Closure requires reproducible test verifying stability.

Triage
Confirm reproducibility with ΔS(question,retrieved) and λ stability check.

Escalation trigger
- Failure repeats after fix attempt.
- Bias, hallucination, or data misuse with risk impact.
- Structural bug outside local operator scope.
