Escalation and Governance — Guardrails and Fix Pattern

This page defines how AI incidents and unresolved failures should be escalated, and how governance boards enforce compliance, accountability, and structural fixes.
It ensures that no bug or bias can stay hidden, and that every systemic failure is linked back to a permanent WFGY solution.


When to use this page

  • Incident or bug repeats despite local fixes.
  • Users escalate “model is unsafe / biased / opaque” issues.
  • Regulatory or audit deadlines require traceability.
  • Stakeholders demand oversight board review.

Acceptance targets

  • All escalations include ΔS, λ, and traceability logs.
  • Time to escalation ≤ 48h from repeated failure.
  • Governance board review within 7 days.
  • Root cause mapped to permanent fix in Problem Map.
  • Closure requires reproducible test verifying stability.

Escalation workflow

  1. Detection
    Incident flagged by eval probes, logs, or user complaint.

  2. Triage
    Confirm reproducibility with ΔS(question,retrieved) and λ stability check.
    If non-reproducible, open debug-playbook.md.

  3. Escalation trigger

    • Failure repeats after fix attempt.
    • Bias, hallucination, or data misuse with risk impact.
    • Structural bug outside local operator scope.
  4. Governance board review

    • Present logs, traces, ΔS, λ, and evaluation reports.
    • Link failure mode to WFGY Problem Map module.
    • Decide on structural remediation and policy changes.
  5. Closure

    • Verify with reproducible test case.
    • Publish fix log with reference to Problem Map.
    • Update governance registry.
