* refactor: remove commands.ts compatibility shim and fix stale references
- Delete packages/cli/src/commands.ts shim file (only re-exported commands/index.ts)
- Update index.ts to import directly from ./commands/index.js
- Update 24 test files to import from ../commands/index.js
- Fix stale CLAUDE.md reference to commands.ts
- Fix stale QA prompt references to commands.ts and wrong line numbers
- Bump CLI version to 0.15.8

Update stale comments referencing the monolithic commands.ts (replaced
by commands/ directory in #2095). The shim at commands.ts still exists
for backward compat but all internal code paths now live under commands/.
- Fix test file comments: point checkEntity to commands/shared.ts,
cmdInteractive to commands/interactive.ts, cmdRun paths to commands/run.ts,
cmdUpdate to commands/update.ts, cmdCloudInfo to commands/info.ts,
cmdListClear to commands/list.ts
- Fix guidance-data.ts: update stale extraction comment and circular-dep
notes to reference commands/run.ts instead of commands.ts
- Fix CLAUDE.md file structure diagram: show commands/ directory and
note commands.ts is a compatibility shim
- Fix packages/cli/README.md: update directory structure diagram and
"Adding a New Command" guide to reflect the per-module layout

Restructure the repo as a Bun workspace monorepo:
- Move cli/ → packages/cli/
- Create packages/shared/ (@openrouter/spawn-shared) with type-guards and parse utilities
- Add root package.json with workspace configuration
- Update all CLI imports to use @openrouter/spawn-shared
- Deduplicate toRecord/toObjectArray helpers from 4 cloud modules
- Update SPA (slack-bot) to use shared package instead of local toObj()
- Update 48 agent shell scripts for new packages/cli/ path
- Update install.sh, install.ps1, e2e, and test scripts
- Update all GitHub workflows, .gitignore, pre-commit hooks
- Update CLAUDE.md, README.md, and skill prompt references
- Pin all dependency versions (no ^ ranges)
- Bump CLI version 0.9.1 → 0.10.0
All 1908 tests pass. Lint clean. All 8 cloud bundles build.
