refactor(review): renumber steps from sub-steps to sequential 1-11

Browse source 
Replace confusing sub-step numbering (1, 1.1, 1.5, 2, 2.5, 2.6, 3,
3.5, 4, 4.5, 5) with clean sequential numbering (1-11).

Mapping: 1→1, 1.1→2, 1.5→3, 2→4, 2.5→5, 2.6→6, 3→7, 3.5→8,
4→9, 4.5→10, 5→11

Updated all cross-references in SKILL.md, user docs, and PR description.
