Windows write_text materializes CRLF, while mutation_evidence reads normalized text; the retry eligibility used raw bytes and therefore denied Windows its one allowed retry. Use the same normalized seed comparison on every OS and pin it with a CRLF regression test. Strengthen the live prompt to require a filesystem tool action and read-back verification, and run the Codex lane on gpt-5.4 medium: low effort twice returned successful prose without editing. The single bounded retry and all hard-red failure classes remain unchanged; a final no-edit includes a bounded answer preview for diagnosis.
