lordratner 5d7f6690c5 fix: active memory recalls stale history in large sessions (#149180) ... * fix(active-memory): keep recall scoped to admitted request Rebuild the request-boundary fix against current main while preserving Codex projection behavior and existing timeout semantics. * fix(active-memory): retain admitted requests through runtime refresh Keep one recorder-owned input representation through prompt rebuilds, preserve no-recorder behavior, and separate deterministic recall reuse by admission identity.
