Peter Steinberger 07e3855b46 refactor(heartbeat): remove legacy HEARTBEAT.md runtime fallback and template repair (#113131) ... The database scratch migration (3e2b3ea4) left two named upgrade bridges: the revision-0 read-only HEARTBEAT.md fallback in the heartbeat runner and the doctor heartbeat-template repair contribution. This deletes both; the doctor scratch migration remains the sole upgrade path.
