Output was artificially capped at 65536; StepFun docs set max_tokens
default to INF, bounded only by the 256K context window. Raise
step-3.7-flash maxTokens to 262144 on both stepfun and stepfun-plan,
matching contextWindow. Per models.dev anomalyco/models.dev#1903.
step-3.5-flash entries unchanged.

* fix(stepfun): zero step-plan step-3.7-flash cost for plan billing

Step Plan endpoint bills per subscription plan, not per token, matching
the sibling stepfun-plan/step-3.5-flash and step-3.5-flash-2603 entries
which already carry zero cost. Standard stepfun/step-3.7-flash keeps its
per-token rate. Per models.dev anomalyco/models.dev#1903, whose step-plan
providers omit cost entirely.

docs: rewrite published docs grounded in current source (#100142)
Source-grounded rewrite of 529 published docs pages with per-unit information-loss verification: 1,713 factual corrections cited to src/**, generated surfaces regenerated, frontmatter titles preserved for i18n, release notes pages untouched. All docs gates green.

fix(anthropic): fall back to Claude Opus 4.8 when Fable 5 safety classifiers decline a request (#99906)
Claude Fable 5 requests on direct Anthropic API keys now opt into Anthropic's server-side fallback (server-side-fallback-2026-06-01): a safety-classifier decline is re-served by claude-opus-4-8 inside the same call instead of failing the turn with "LLM request failed.". Mid-stream boundaries drop the declined model's thinking/tool blocks per Anthropic's replay contract, keep the partial text as the continuation prefix, record a provider_fallback diagnostic, and cost the turn at the serving model's rates. Docs lead with the coupling: using Fable 5 means also using Opus 4.8. OAuth, proxies, Bedrock, Vertex, and Foundry requests are unchanged.

Live-verified: a benign reasoning_extraction classifier decline through the product path returns an Opus-served answer with the provider_fallback diagnostic; exact-head CI green (67 checks).

feat(xai): route OAuth login through device-code flow (#97249)
Route xAI OAuth through device-code sign-in so remote and headless hosts do not need a localhost callback. Preserve the legacy manual `xai-device-code` auth choice/method as a compatibility alias to the same device-code flow.

Also migrate stale xAI token endpoints on refresh and fail fast on structured refresh errors while keeping retries scoped to detected HTML/Cloudflare challenge responses.

Verification:
- `node scripts/run-vitest.mjs extensions/xai/index.test.ts extensions/xai/xai-oauth.test.ts`
- `node scripts/run-vitest.mjs src/cli/models-cli.test.ts -t 'maps --device-code'`

- Crabbox local-container live smoke on exact head `fef3cb24afb01cd1f69cf04ef67ed11d71dfadb3`: xAI discovery and device authorization returned 200.
