[SMF/UPF] Changes subnet configuration (#2975)

The way subnet is set up has changed as shown below.

```
<OLD Format>
smf:
  session:
    - subnet: 10.45.0.1/16

<NEW Format>
smf:
  session:
    - subnet: 10.45.0.0/16
      gateway: 10.45.0.1
```
