git grep defaults to searching only tracked files, causing code_search to miss untracked directories like internal/mcp/. Add --untracked flag in workspace mode so both tracked and untracked files are searched. Also fix non-git-directory detection to use exit code 128 instead of hardcoded English error string, making it locale-independent.
