Alessandro a13262324c Require lifecycle hooks for plugin setup and cleanup 
Make hooks.py mandatory for dependency installation, required initialization,
and uninstall cleanup in plugin authoring and management guidance. Remove
the execute.py setup example and flag manual lifecycle steps as review
failures, while preserving shared dependencies and unrelated user data.

Align the development reference and owning DOX contracts. Validate skill
discovery in the live framework runtime, relative references, Markdown
fences, and Python example syntax.
